Transfer data after logged into server post Minecraft Profile name change

Hi,

I recently changed my in-game profile name for Minecraft. When I log into the direwolf20 1.12 server after the name change it seems like the server treats me as a new person. Is there anyway I can add my data from previous name change to new one? (i.e. homes, items etc.)

My previous in-game name: Dylforthewin14 (registered with server)
My new in-game name: Sil3ntCha0s (unregistered with server)
