no grave spawned

I was replaning my Power room and picked up my ic2 reactor and pick up a fuel rod whit out the Hazmat Suit and died...

i was going back to the point whit the /back command and the reactor was fine (just like i would never touched it) but no grave stone spawned and i lost my items...

i dont know if a refund is possible because i dont have a real proof...

Just in case the  most expensive items i had were a full nano suite and a mfe...
