Game Crash Frequently When Teleport To The /home Location

From yesterday my game crash frequently (not everytime) when I teleported to /home location by using Enhance chamber of dislocation or by /home. I've reinstall my game + use other launcher but the problem still not be fixed. I attach 3 newest crash log (I forgot to save others when I reinstall my game). Can somebody find out what is happening?

I just disabled it, I though this modpack don't use fastcraft anymore. I will test it tomorrow
