Being teleported into the clouds when i jump and phasing thru walls and ceilings

Every now and then I'll jump and get teleported into the clouds. Usually a relog would fix this. Today I relogged 4 times. Reloaded the modpack twice. And now Im not only being teleported into the clouds but I'm also phasing thru ceilings and walls when i jump and im getting stuck in them and suffocating. Some help would be nice
