Description of Issue: I went to the nether, then died after just some seconds due to lag and a mob. Came back immediately after and the grave was not there. Also everything i just mined or placed was not there (like a world reset). I foundout that this happens every time i go to the nether. If i set a new home there, also the portal disappears. (this is definitely a server bug)

PS: Into the satchel there was a lot of things. The most important was the philosopher stone. But i can provide a full list if needed
