A Payment Issue Template

Before I say my idea, I need to clarify something. This is supposed to be for all servers, in the technical support category ( I couldn't find a suggestions page for all servers). So my idea is we put a Payment Issue Template in the technical support category, I see this need because I always see players complaining about Payment Issue problems and I think there should be a template, and it should look something like this;

In game name:

Transaction Number (Optional):

Description of Issue: 

Date/Time of purchase:

Item bought:

Screenshots (Optional):

Any other important information:

I feel like it would make handling these issues easier with all the information right there, rather then asking and waiting for replies.

Actually I like the first template better, we could definitely use this across all servers.
